By getting to know our clients and their families, we can create an individualized treatment plan and environment to target your specific goals and priorities.
The information listed below will allow our team to determine services your child may need, begin the insurance funding process and assist us in preparation for an initial assessment.

Medical Report
Be sure you have a copy of your autism diagnosis report from your physician. If you have not gotten an official diagnosis, we can offer autism testing / autism diagnosis services.

Referral Letter
Get a letter from a doctor (primary care physician, pediatrician, neurologist, etc.) recommending ABA therapy as a medically necessary treatment.

Research Insurance
Contact us to verify your insurance benefits. Our insurance specialist will work with you to determine the details of your insurance policy’s coverage for ABA therapy.

Intake Form
Complete and submit the intake form and attach any other required documents.

Begin Services
We will schedule an initial consultation to discover your goals for treatment and begin customizing a plan to suit your family’s lifestyle and ensure the best results for the client.
